This downloadable application is for any one willing and able to help at the pantry with pick-up and delivery of food once you have completed this form and emailed or mailed it please be patient with us. We are excited to have you help but first we need to review your application and contact you. Hang in there we will be in touch and in the mean while go to the Get Involved page to sign up and sharing your availability with our coordinator.

Assistance Application

This application is for families living in Green, Jackson or Stoney Creek Townships surrounding Lapel Community once you have mailed or emailed your application you will be contacted to confirm your next allowed food assistance date.
